Clinical Review of Whipple Operations



Abstract
We evaluated 51 patients who received Whipple operation at Hanyang University Hospital from June 1979 till May 1991.
@ES The result were obtained as follows;
@EN 1) Among 51 cases, 18 cases were carcinoma of the head of pancreas, 12 cases were cancinoma of the ampulla 15 cases were carcinoma of the distal bile duct, 2 cases were carcinoma of the duodenum, and one case was advanced gastric Ca.
2) The duration of average postoperative admission was 25 days.
3) Postoperative complication were developed in 16 patients(31.3%) Among 16 cases, 5 cases(9.8%) were non-specific pulmonary complication and 4 cases were(7.8%) wound infection.
4) There were 3 cases(5.8%) with gastrointestinal hemorrhage.
5) These were 4 cases(7.8%) with fistula at choledochojejunostomy site, but there was no fistula formation at pancreaticojejunostomy site.
6) Hepatic failure & renal failure that were directly related with jaundice were not developed.
7) There were three operative mortality. The first case was expired due to sepsis after emergency operation, since bile peritonitis was developed after PTCD. The second case was expired at 2 wks after operation due to multiple organ failure. The
third
case with advanced gastric ca. Was expired at 2 days after operation due to massive intraabdominal bleeding.
8) There was no reoperation, and was no diffrenece in incidence of complication between location of cancers.
9) Expected 5 yr survival rate was 5.5% with carcinoma of pancreatic head, 50% with carcinoma of distal CBD, and 64% with carcinoma of ampulla of vater.
